Ну, учитывая сколько лет изучается сифилис и боррелиоз, то до создания диспансеров и скрининга осталось лет 50-100:). Хотя лучше так:(.
Так собственно весь форум об этом, хотя хотелось бы побыстрее, мы столько не проживем. o_O Тем более, что и возможности науки с тех пор намного усилились, процесс можно было бы ускорить при желании.
Потому "оптимистические" высказывания некоторых врачей не находят здесь понимания.

Как я себе понимаю основное противоречие между ИДСА и ИЛАДС он в том что и те и другие толком не знают как достигнуть полного изличения. В ИДСА руководствуются тем что в 90% случаев терапия 2-3 недельними курсами аб дает ту самую ремиссию. И соотношение риск\ ремиссия на уровне 3 недель оптимальное для большой выборки . В ИЛАДС в свою очередь терапия расчитана на воздействие на все формы но эффективность этих схем тоже не особо точная те же БЦА говорят о 60%. Отсюда общие риски от терапии помноженые на сроки плюс избыточная диагностика. Я вот например до сих пор не понимаю смысл диагностики Коксаки вируса в БЦА в отсуствие какого либо лечения от него.
С другой стороны у ИДСА явный какой то мухлеж с хронической формой. То есть формально статистический эти люди есть речь о том что нужны более детальные исследования есть. А исследований нету. Притом сами пишут что что и терапия пеницилинами приводит к хронизации и что есть часть популяции у которых сохраняются симптомы. И в принцепе признают без эритемную форму. Но ответов как это лечить кроме повторить еще один раз 3 недельный курс а дальше лечить симптоматический. Из за отсуствия вменаемой схемы действий в ИДСА с хрониками другой алтернативы кроме как в Иладс вроде как и не видно. Что в общем странно так как те данные есть даже на руках у классической медицины очевидно что хроническое течение бывает.

Что касается ко инфекции их ни кто особо не отрицает все зависит от квалификации лечащего врача. На много интересней все в области оппортуническими инфекциями типа микоплазмы, хламидии пневмонии, герписов и тп. Классическая медицина это вообще не увязывает с боррелиозом кто тут прав хз. Лично мне позиция что боррелия может быть причиной ослабления иммунитета и как следствие активации этих инфекции кажется логичной. Но доказательств на сколько это все связано кроме опыта форумчан я пока не встречал кроме статистических наблюдений. Хотя наверно роль боррелии в обрушении иммунитета не так уж сложно исследовать.

В остальном разногласия не так уж велики если брать граммотных врачей из обоих школ основная проблема имхо с общей безграмотностью врачей и в той и другой школе. Потому что сколько в ИДСА идиотов столько же в ИЛАДС шарлатанов с разными сомнительными практиками.

И тут конечно было б еще интересно поговорить о диагностики потому что у ИДСА проблема в том что нету стандарта ни блот ни ифа а те требования по интерпритации которые есть не сколько радикальны избыточно я б сказал в этой статье по 5 белкам положительный результат если я ни чего не путаю сейчас вроде смягчились чутка требования. У ИДСА забавно все данные за то что хронический боррелиоз есть. Но как его диагносцировать и лечить ответа толком нету.

С другой стороны у иладс и ЛТТ как анализ вызывает много вопросов да СД57 тоже очень статистический анализ. Интересно б конечно узнать было почему с точки зрения классической медицины эти анализы не показательны.
Я так понимаю что у ЛТТ есть проблемы с ложно положительностью а понижение сд57 чисто статисческое наблюдение бывающие при многих других состояниях.

Конечно очень хотелось бы открытой дисскусии между этими двумя взглядами и как итого какой то обще взвешенной линии. А то пока каждый при своем пациенты находятся в ваукуме и как лечится и что делать приходится решать каждому индивидуально что вообщем проблема.

Randomized controlled studies of treatment of patients who remain unwell after standard courses of antibiotic therapy for Lyme disease are in progress. To date, there are no convincing published data that repeated or prolonged courses of either oral or iv antimicrobial therapy are effective for such patients. The consensus of the Infectious Diseases Society of America (IDSA) expert-panel members is that there is insufficient evidence to regard “chronic Lyme disease” as a separate diagnostic entity.

Ах, любимые объективные доказательства... Это которые? На каких тогда построена называемая рекомендациями писанина?
Если в системе доказательств хоть одна составляющая кривая, то вся система будет кривой, а значит это не доказательства.
Если неподходящим антибиотиком, названным подходящим на основании выбранных (не всех!) фактов, проводить продленные курсы лечения, то конечно же такие лечения не будут приносить результатов!
Вся эта конструкции держится на основании схемы лечения боррелиоза в 2-3 недели, которая вообще не имеет под собой доказательств и обоснований, связанных с реальностью, включая морфологию патогена. Однажды я смотрела исследования с доксициклином, на основании которых выстроена вся эта громадина. Это исследования низкого качества (так они называются/оцениваются), где в питательную среду с бактериями добавили доксициклин, а потом посчитали оставшиеся спирохеты. Спирохет там не было. А скрытые формы, в которые они перешли, никто не считал. Считали только спирохеты. Вот это основа и точка отсчета всей конструкции. Потом этим лечат две (неизвестно на каких основаниях) недели людей, им вроде лучшает - и в дамки. Про потом речь не идет.
О каких объективных доказательствах вообще может идти речь, если не существует способов объективного выявления заболевания? Достаточно посмотреть на тесты, которые "выбрала" себе в "помощники" медицина - сами же говорят о 60% вероятности. С такими тестами вообще не может быть доказательств: то есть если явное "да", то да, а если нет, то в дело вступают заботливо подготовленные манипуляции.
Вся эта каша заварена методом частичной правды: повыбирали удобные факты и слепили, что надо, добавив воды. Вот яркий пример и подтверждение (одни из!), (про ваш любимый цефтриаксон, который "самый лучший" и продленное лечение им):

"A key component of the monkey study recently published by Embers and colleagues was a 90 day treatment trial designed as a companion to the highly-cited Klempner human trial of chronic Lyme disease published in 2001. The treatment regimen in the Klempner companion monkey study was exactly the same as those used for the human study. Unlike the human trials, however, the monkey trials were able to directly demonstrate persistent infection. In fact, the Embers study was proposed by patients on the Klempner advisory committee to act as a fact check on the results of Klempner, with the expectation that the two would be published in tandem. It is big news for patients that the two trials reached opposite conclusions.


The monkey trials found that the bacteria that cause Lyme disease persist after 90 days in monkeys treated for chronic Lyme disease. Further, the antibody tests used to diagnose Lyme fail to detect disease in late Lyme at least 50% of the time. Why are these findings important? Well, the 2006 Infectious Diseases Society of America (IDSA) Lyme guidelines rely heavily on the Klempner study to deny persistent infection and, consequently, antibiotic treatment for patients. After all, why treat an infection that doesn’t exist? The IDSA guidelines also require that patients have a positive lab test to be diagnosed (and treated) for Lyme disease—lab tests that Embers shows miss 50% of late Lyme disease cases.


But this is all old news, right? I mean the monkey study was funded in 1998. But, wait—the results of that study weren’t published until 2012. Does that type of a delay in publication matter? You bet it does. Here’s why. Science builds upon existing published research. New research incorporates the work of others into a new hypothesis, refines it, and sometimes transforms it.

Leaders in evidence-based medicine, like Dr. Iain Chalmers, point out that “Peer review cannot take account of what cannot be seen.” Chalmers explains that unreported research stops scientific progress dead in the water and wastes precious resources vital for patient care and research. Further, it can form the basis for seriously misleading recommendations leading to unnecessary suffering and death. A recent editorial in the British Medical Journal called the crisis of missing trial data a threat to the integrity of evidence-based medicine; they concluded that “[a] current culture of haphazard publication and incomplete data disclosure make the proper analysis of the harms and benefits of common interventions almost impossible for systematic reviewers.”

How does this apply to Lyme disease? Imagine that a scientific result has a bearing on the treatment of a disease. Imagine that the publication of that result would relieve unnecessary human suffering but is delayed for 10 or more years. Imagine then, that these results are not reflected in clinical guidelines, leaving patients inappropriately treated by their doctors for want of knowledge of that scientific result. And that is what essentially happened here, as the time line below shows:

So the Klempner study and the companion monkey study were funded within two years of each other. But while the results of the Klempner trial on humans were rushed to publication, those of the companion monkey trial languished for another for 11 years. During this period, seriously ill patients were told there was no persistent infection to treat, and physicians who treated patients for persistent infection were brought before medical boards.

I found myself wondering where the NIH – which funded and oversaw the Klempner companion study – had been for the past ten years. The study was funded and monitored under the watch of Dr. Phil Baker, who has now retired from the NIH and advocates against Lyme patients on behalf of the American Lyme Disease Foundation.

Baker denies allegations that the NIH suppressed publication of the study and claims the decision to delay publication rested entirely with the authors. There are several possible explanations for the delay. It is possible that publication was delayed to ensure the completeness of the study. It is also possible that after the Klempner trial was published and the nature of the Lyme debate became so heated, the political headwinds against publication of contradictory results simply became too challenging. If so, the publication even at this late date might be regarded as an act of courage on the part of the authors. There can be no doubt, however, that the delay in publication caused patients considerable harm.

Разве Вы и Ваша жена не являетесь доказательствами эффективности терапии по ILADS? )

Вы забываете о том, что у IDSA и ILADS пациенты разные. ILADS занимается в подавляющем большинстве пацентами с запущенной болезнью и от которых уже отказались лечащие по IDSA. Это несравнимые категории.

Around 80-90 percent of all treated patients got improvement during or after treatment, meanly antibiotic longterm treatment. Between 60 and 65 percent of them got full improvement. Full improvement means no further activity in the cellular tests ( Elispot Borrelia) and improved CD 57 NK cells and additionally improvement for the clinical signs and complaints.
Around 10 percent of patients did not react on treatment neither antibiotics nor herbal anti-infectious treatment (alternative protocols)

2. Is there any statistics on the follow-up, e.g. how many percent that experience relapse after 1 year?
Between 10 and 20 % of the treated patients got a relapse within the first year.

3. What reasearch do you refer to when giving long-term treatment, that we as association also can point to when we get questions?
In general we treat all patients only according the the International Guidelines of ILADS (http://www.ilads.org/lyme_disease/about_lyme.html) and the German Borreliosis Society (http://www.borreliose-gesellschaft.de/T ... elines.pdf) Both of them are evidence based.

Удивительная логика - если не понимаю, значит это что-то негативное, например - избыточная диагностика.) То, что вирусы влияют на состояние иммунитета, Вы не знаете? И что симптомы могут быть похожи. И что при определенном статусе иммунитета и активных вирусах антибиотики не назначают?

LYMEPOLICYWONK: Embers Monkey Trials Part 2: Chronic Lyme Disease Treatment and Persistence | LymeDisease.org

This is Part 2 of a series on the Embers study of Lyme disease in non-human primates. As described in Part 1 of the series, the Embers monkey research study posed three questions: one regarding treatment of early disseminated Lyme disease, one regarding treatment of late disseminated Lyme disease, and one regarding the ability of the C6 to accurately detect infection. This part of my blog series on the Embers study focuses on the second question–the ability of 90 days of antibiotics to eradicate infection in late disseminated Lyme disease. The researchers defined late disseminated Lyme disease as 27 weeks after inoculation. Rhesus macaques were chosen as the animal model because they experience many of the key signs of human Lyme disease, including neuroborreliosis ( an infection of the brain or nervous system.)

Parallel Studies: The study was intentionally modeled after the 2001 NIH funded Klempner trial, which assessed the effectiveness of retreatment of patients with chronic Lyme with 30 days of IV Rocephin followed by 60 days of doxycycline. The Klempner trial looked at the SF-36 quality of life assessment and found the 90 day treatment protocol did not significantly improve patients’ scores.The treatment protocol in the Embers study was identical to Klempner but had the advantage of being able to use invasive tissues sampling to determine persistence—something which cannot be done in human subjects.

The idea of conducting a parallel study in monkeys so that actual persistence of infection could be determined through invasive tissue sampling was specifically proposed by the advisory committee to fact check the human results. (Carl Brenner of the National Research Fund for Tick-Borne Diseases and Phyllis Mervine, President of LymeDisease.org were both patient members of the Advisory Committee.) The hope was that the results of the two studies could be published at the same time. Unfortunately, for reasons unknown, the publication of the findings of monkey study–which contradict those of the Klempner study–was delayed for 11 years. (The Klempner study was funded in 1996 and completed in 2001; the portion of the Embers monkey study dealing with chronic Lyme was funded in 1998.)

Study Design: The chronic Lyme protocol in the Embers trial involved 24 monkeys, which were infected with Borrelia burgdorferi (Bb), and 4 control monkeys, which were not infected. Of the 24 infected monkeys, 12 were treated with antibiotics, 12 were not. One monkey in the antibiotic group of the study died of unrelated causes. Of the 4 uninfected control monkeys, 2 were treated with antibiotics and 2 were not.

The timeframe of the study involved infecting the monkeys with Bb, waiting 27 weeks, and then treating them for 90 days as in the Klempner study. Six months after treatment, intensive tissue sampling was conducted to determine whether Bb persisted notwithstanding treatment.

Results: 8 of 11 infected monkeys had persistent infection notwithstanding treatment. This means that for monkeys that go untreated for 4 months before antibiotic regimen begins, persistent infection after 90 days treatment is the norm. And it suggests that other treatment options should be explored. These could include longer treatment durations like those used in other chronic infections such as tuberculosis (2 years), different antibiotics, combination antibiotics, and pulsed therapy.

The fact that the Klempner study and the Embers study conflict is important because the IDSA guidelines rely on the Klempner study to deny patients treatment for chronic Lyme disease. In its guidelines, the IDSA contends that the Klempner study showed antibiotic treatment was ineffective on patients with chronic Lyme disease. This contention relies on a premise that Embers proved false—namely that 90 days treatment with antibiotics would be sufficient to clear infection. The Embers study shows that this assumption is wrong. The infection persists even with 90 days of treatment. This suggests that rather than being ineffective as Klempner concludes 90 days of treatment may be insufficient to clear the infection.

Another important factor to keep in mind is that Klempner did not try to establish persistence through direct evidence from tissue testing, like Embers. Instead, the Klempner study attempted to measure effectiveness of antibiotic treatment on humans indirectly using the SF-36 quality of life survey and found that patient responses did not improve after 90 days treatment.The Klempner study has been criticized because the sample size was too small to measure clinically relevant treatment effects. However, beyond this, if, like the monkeys, the patients had persistent infection after 90 days of treatment, the treatment may not have been sufficient to improve patients’ quality of life.

The Klempner study went further though and concluded that there was no evidence of persistence based on negative blood PCR and blood culture—both regarded as highly insensitive (but non-invasive) methods to look for persistence. In contrast, the monkey trial relied on evidence from tissue samples. This contradiction underscores the importance of using animal models for research. The Embers method of demonstrating persistence, invasive tissue sampling, would be impossible to conduct on humans. In fact, this was the reason the parallel monkey trial was conducted.

Significant excerpts from the study:

Persistence: “Our results indicate that disseminated spirochetes of two different B. burgdorferi strains can persist in the primate host following high dose, or longlasting antibiotic therapy.”
. . . . “Our studies do however offer proof of the principle that intact spirochetes can persist in an incidental host comparable to humans, following antibiotic therapy.”
. . . . “Finally, the use of variable and pulse-dosing regimens of antibiotics may improve efficacy [43] and this warrants testing in an appropriate model.”

Mechanisms of Persistence: “The nature of the persistent organisms and the acquisition of tolerance to antibiotics are questions that need to be addressed. The B. burgdorferi spirochete is known to invade collagenous tissue as a possible mechanism of immune evasion.”
. . . .”The fact that organisms can persist in the presence of antibiotics such as penicillin and cephalosporins (ceftriaxone) that interfere with cell wall synthesis appears to stem from their ability to enter a dormant, non-dividing state [43,44], thus avoiding the need for cell wall synthesis to continue growth.”
. . . .”A ‘‘persister’’ phenotype may possibly be responsible for the recalcitrance of persisting spirochetes made evident by previous studies in mice and dogs [37,42,45], and by those presented in this report.”

Гепати В, гепатит С, лепра, туберкулёз, ВИЧ-инфекция - курс лечения далеко не несколько недель. Некоторые хирургические инфекции также могут лечиться месяцами и годами.



Положительная РВ совсем не означает наличие спирохет и тем более сифилиса. Например, при беременности, туберкулёзе, опухолевых процессах, аутоиммунных заболеваниях и т.д. часто встречаются ложноположительная реакция. Поэтому сейчас она применяется только как скрининг и для постановки диагноза обязательно подтверждается другими исследованиями. К тому же есть понятие серонегативного сифилиса.
Я думаю, что тут дело в достаточно устаревших методичках, которые никто особо не хочет пересматривать.
У вас все смешалось! При чем тут вирусные инфекции?! Совсем другой патогенез, другие принципы лечения. Я уж молчу, что ВИЧ и хронический гепатит В на сегодня просто не излечимы. А уж ваши выводы о методичках может делать только человек, который вообще в теме. Ну напишите сами иетодичку. Вам- то кто мешает?:(

Юль я всегда в споре иладс vs идса был обоеми рукамиа иладс . Лично я со своими предпочтениями давно уже определился но тем не менее у ИЛАДС тоже есть некоторые неочевидные подходы и открытая дисскусия эту тему крайне полезна. ИЛАДС не до конца решил мою проблему пока он конечно сильно улучшил состояние но вопросы есть. :)
Что касается момента с вирусом то при 100 кратном превышении антител ,антибиотики мне прекрасно назначали и внятно обьяснить зачем на это сдаю не смогли. Хотя я допускаю что это мой конкретные проблемы с врачом к сожалению они как и в идса не все одного качества. Но по мимо этого у меня было еще как минимум 2 опыта с откравенными шарлатанами с рекомендации из иладс. Тут как то надо различать научную базу и подходы и реалии. В Иладс к сожалению с отбором не так все хорошо.
Что касается исследования спасибо вам за статью в принципе статья с мышами примерно об этом же. Меня собственно тоже очнь настораживает в ИДСА манипуляции с реальностью и их попытки сделать вид что хронической формы нету я собственно об этом тоже написал.:) Другое дело что вот недавно была статья постил Андей о том что испробавана схема в 3 антибиотика с доптамицином которая вроде как действует на все скрытые формы. В той же статье речь шла что они протестировали все двойные схемы и не одна из них не приводила к полной элименации патогена.
Проблема в том что нету пока 100% метода уничтожить все патогены. И те и те в основе своей занимаются ремиссии ИДСА счетает что их метод работает для 90% насиления и относительно безопасен. Если же сразу лечить всех схемами иладс руководствуясь их логикой то это миилионы людей принимающие длинные курсы аб месяцами годами. Это проблему резистентности к аб перенесет сразу на другой уровень. Это какой то процент случаев аллергии на аб и смерти связаной с этим. И это какое то колличество врачебных ошибок. И запустить все это стоит не меряных денег. При этом статистики по смертности от лайм нету а от аллергии на аб и лекарственных геппатитов будет. Все равно будет оставаться % не вылеченых требующих лечения годами. С ведением такого заболевания которое можно лечить годами а потом некоторым надо будет пересаживать почки печень ценна страховки для всего насиления выростит. Сколько пока получится с экономить в отсуствие доказано работающей терапии, диагностики вычеслить не представляеется возможным. И по скольку все таки государство должно решать проблему не только связанные с боррелиозом. И смертность от той же клостридии устойчивой к аб в сша 60к в год и она растет по экспаненте. По этому почему ИДСА у руля мне в принцепе понятно. Меня вот только очень смущает открытый мухлеж в плани хронической инфекции очень сложно верить организации которая так усилено врет в таком моменте. Я конечно понимаю что из за хрони там стройная картинка не сходится. Но зачем так грубо то...
@Hellena вот вам близка позиция ИДСА может вы нам проясните отчего такое искажение реальности с хронической персистенции боррелиоза и почему такое отрицание скрытых форм боррелии ?

Просмотрела еще раз эту грамоту, уже в который... Действительно, в этот раз читается уже бегло. :) Но вопросов возникает все больше.:cautious:

Ну явно же накручено! Еще надо смотреть сами испытания: сколько участников, как проводили, какими методами оценивали, как и на каких основаниях сделали выводы... Глаз да глаз!
В рекомендациях 405 ссылок, а по ним будет еще в среднем по 100 на каждую, а в тех еще по 50, и так далее. Там так закручено, что концов не найдешь, даже если все исследования будут лежать на столе перед глазами... Попробуйте еще их найти! Не больно-то спешат выклаывать в открытый доступ. Почему, спрашиватся..? Что ж там такого может быть, чтобы человек не мог прочитать, на каких основаниях его лечат? :sneaky:

Еще нужно смотреть как они вообще выбирали все антибиотики! Совсем не исключено, что это было так же, как в том исследовании с доксициклином, о котором я писала ниже. На нем были построены старые шведские рекомендации, в частности.

Все-таки самой большой загадкой остается вопрос на каких основаниях они "рекомендуют" лечение в 10-14 дней (или 21)... Везде только констатация и никаких обоснований - почему 10-14. На каких основаниях считается, что этого достаточно?!
Ведь от первоначальной диагностики и исхода терапии ой как многое зависит...

Я даже не сомневаюсь, что это будет "смешно". Такой же невероятный каскад водоворотов с 600 ссылками под ним...

Вообще, неужели вы думаете, что все эти светила, которые составляли рекомендации и понаписали книг на 700 страниц, не знают, как боррелия (и другие бактерии!) в присутствие антибиотиков сворачивается в цисту за одну минуту, а позже, в комфортной среде, может снова переходить в спиралевидную форму? Помилуйте, но это видят все исследователи в микроскоп своими глазами! И все это описано еще в начале прошлого столетия и многократно после! (МакДоналд, в частности, написал несколько книг со всеми доказательствами, но его отказываются публиковать в научных журналах.)
Поэтому рекомендатели не могут этого не знать! Так же, как и не знать о морфологии патогена, с которым они рекомендуют бороться антибиотиками и курсами, которые не могут привести к эрадикации, в лучшем случае организм больного сам законсервирует инфекцию. А после этого притворяются, что сомневаются, будто инфекция может остаться...
Слушайте, но это ж цирк... Для того, чтобы подтвердить, что инфекция остается, не нужны какие-то анализы. Она и так остается!

Они всё знают. И знали с самого начала.
Поэтому эти рекомендации имеют целью оставлять в организмах как можно больше инфекции, но при этом обязательно делать вид, что излечение происходит.

Azithromycin Compared with Amoxicillin in the Treatment of Erythema Migrans: A Double-Blind, Randomized, Controlled Trial

Objective: To determine whether azithromycin or amoxicillin is more efficacious for the treatment of erythema migrans skin lesions, which are characteristic of Lyme disease.

Design:
Randomized, double-blind, double-dummy, multicenter study. Acute manifestations and sequelae were assessed using a standardized format. Baseline clinical characteristics and response were correlated with serologic results. Patients were followed for 180 days.

Setting:
12 outpatient centers in eight states.

Patients:
246 adult patients with erythema migrans lesions at least 5 cm in diameter were enrolled and were stratified by the presence of flu-like symptoms (such as fever, chills, headache, malaise, fatigue, arthralgias, and myalgias) before randomization.

Intervention: Oral treatment with either amoxicillin, 500 mg three times daily for 20 days, or azithromycin, 500 mg once daily for 7 days. Patients who received azithromycin also received a dummy placebo so that the dosing schedules were identical.

Results: Of 217 evaluable patients, those treated with amoxicillin were significantly more likely than those treated with azithromycin to achieve complete resolution of disease at day 20, the end of therapy (88% compared with 76%; P = 0.024). More azithromycin recipients (16%) than amoxicillin recipients (4%) had relapse (P = 0.005). A partial response at day 20 was highly predictive of relapse (27% of partial responders had relapse compared with 6% of complete responders; P < 0.001). For patients treated with azithromycin, development of an antibody response increased the possibility of achieving a complete response (81% of seropositive patients achieved a complete response compared with 60% of seronegative patients; P = 0.043). Patients with multiple erythema migrans lesions were more likely than patients with single erythema migrans lesions (P < 0.001) to have a positive antibody titer at baseline (63% compared with 17% for IgM; 39% compared with 16% for IgG). Fifty-seven percent of patients who had relapse were seronegative at the time of relapse.

Conclusions: A 20-day course of amoxicillin was found to be an effective therapeutic regimen for erythema migrans. Most patients were seronegative for Borrelia burgdorferi at the time of presentation with erythema migrans (65%) and at the time of relapse (57%).

Итак, испытаниям подверглись 246 пациентов.
217 из которых получали амоксиллин 5оо мг два раза в день в течение 20 дней.
Сколько пациентов получали азитромицин 500мг один раз в день в течение 7 дней?

Что это означает?:
"500 mg once daily for 7 days. Patients who received azithromycin also received a dummy placebo so that the dosing schedules were identical."
Так сколько из них получали азитромицин и сколько плацебо?

То есть выводы сделаны так: 217 пациентов, 20 дней принимавших амоксициллин 500 два раза в день показали лучший результат, чем неизвестное мизерное количество пациентов, 7 дней принимавших азитромицин 500 один раз в день.
Насколько я понимаю, исследование показывает, что азитромицин был менее эффективен, чем амоксициллин
только при таких условиях.
Но все это трактуется и выдается чуть ли не как глобальная и историческая неэффективность азитромицина для лечения боррелиоза.
Или я чего-то не понимаю? :rolleyes:

Елена вот вы не раз и не два написали о том что нас разводят, курсы аб избыточные и что наших любимых актеров зарубежом за лечили. Все это очень спорные утверждения вызывающие смешаные эмоции. Не могли бы вы все таки приоткрыть завесу тайны. Как там дела у боррелиоз с хронической формой если вы согласны что хроническая форма бывает то чем она конкретно вызвана и почему для лечение этой самой формы длинные курсы аб не подходят.
И такой вот момент если вдруг хронической формы не существует и у боррелии нету скрытых форм то почему вот здесь вот Консультации - тяжелый случай вы не по рекомендовали @eakroha какой то внятной схемы в которая могла бы ее вылечить раз и на всегда от боррелиоза?

А вот тут я просто не сколько озадачен оказывается хинолонолы, тинидазол , флуканазол и пульсы не рекомендованны идса... То есть по идее врач может и по шапке получить за их использование, че то я о такой жести не знал... Что ж они будут делать в свете работы Занга....

Тут вообще бомба. Оказывается к следующим гайдлайнам они рекомендуют поменять, что в случае отсуствия эффекта от терапии не могут рассматриваться, а должны рассматриваться ко инфекции.

Красавцы теперь персистенция оказывается есть но почему то она не вдияет на клиническую картину...
Это уже помоему вершина абсурда)

Вообщем для нас это все не новости, но для людей, которые борятся с официальной медициной на западе, этот документ может быть полезен...

А у меня просто слов нету. Прошло 9 лет, мы живем по старым рекомендациям , а они даже не удосужились эти комментарии внести... Редкие утырки...

здесь ларчик открывается довольно просто, длительная терапия направленная не на того возбудителя конечно же не эффективна, а рассматривать альтернативные инфекции они не могут или не хотят, ведь бартонеллы например не являются в их понимании угрозой и не являются проблемой при лечении боррелиоза, гораздо проще вывести все нерешённые вопросы в постлаймовский синдром да и умыть ручёнки.

Я не знаю у них по моему конкурс...
Глупость штука неискоренимая жаль когда глупость на столько устойчива что переходит на бумагу...

А чего стоит признание идса что скрытые формы есть но нету доказательств что их уничтожение улучшает самочувствие...
Притом что переход из спирохеты в раунд форму и обратно уже только ленивый не доказал..

Дрей вы слишком глубоко копаете:) Это ведь официальная бумага типа как научная статья...
Не мне конечно об этом говорить, но у слов вроде как есть какой то смысл...

Однако можно однозначно утверждать,- если одназначно что то можно утверждать то значит автор работы проверил все возможные аб в разных дозировках и все возможные схемы приема. Конечно можно сказать что я предираюсь, но ведь потом такие же врачи читают такие же бумаги и с умным лицом однозначно утверждают что длительные схемы аб не работают и никто ведь не говорит что все длительные схемы тестировались на цефтриаксоне и доксицилине в моно вариантах....

что длительная антибактериальная терапия положительного эффекта данным пациентам не принесет. - Вот каким то образом это цефтриаксон и доксицилин в моно варианте это вся антибактериальная терапия,
и ведь именно на основе таких заключений врачи и повторяют как роботы, что длительные аб не показаны, хотя понятие антибактриальной терапии не ограничивается тетрациклинами и цефалрспоринами...
Ну и последняя часть не принесет положительного эффекта... Это вообще что такое?) Просто большая часть жалоб больных боррелиозом субъективные жалобы типа головной боли, слабости, брейн фог,депрессии,мигрирующих болей и тп, то есть, эти симптомы известны со слов пациента и не могут быть измерены и зафиксированы .
Если говорить о ИДСА то они как раз улучшение этих симптомов не признают, а так в 2 из 4 самых крупных исследованиях, на которые базируется идса в плане субьективных симптомов у людей было улучшение...

Я уж молчу о работе биостатистиков, указывающих на то, что все исследования составлены
некорректно, и их данные не достоверны в связи с этим.А на чем и на каких исследованиях основывается автор я не знаю, потому, что не вижу ссылки на против этого утверждения на работу, которая поясняет, что такое положительный эффект и о каких конкретно аб идеть речь...

Так что какие тут к черту БЛО и ко инфекции если тут просто у человека за словами ни какого смысла нету...
